This is a funded program that provides nursing supports to transfer payment agencies in the Ottawa area. Our Health Care Facilitator's (HCF) main role is to increase the capacity of agencies to care for individual's who are aging in the home and have increasing complex medical needs.
The HCF supports agencies in navigating the medical sector, advocacy, discharge planning, controlled acts training, clinical assessments to evaluate for continuing supports, immunizations, PCR testing, policy advice, and urgent medical referral support.
This is a fee-for-service program that provides direct nursing supports for person's living in group homes. We support individuals living with transfer payment agencies as well as outside paid resource agencies.
Our Direct Support Nurse (DSN) is very active in discharge planning, consulting with specialists, advocacy, virtual and in-person assessments, wound management, regular injectables, treatment plans, wellness visits, palliative care, EDITH, urgent medical referral support, and controlled acts training.
Our HCF provides free training for controlled acts via webinars for all Ottawa TPA agencies as part of the funded program.
Agencies can also access in person training for controlled acts through our fee-for-service program and our DSN will come out to any agency and teach in-person.
The Ottawa Rotary Home also offers a comprehensive, affordable online, self-paced learning program and practice labs for all controlled acts and advanced care skills through our social enterprise. Community Support Training Solutions offers many courses to meet the needs of developmental services.
