This is a funded program that provides nursing supports to transfer payment agencies in the Ottawa area. Our Health Care Facilitator's (HCF) main role is to increase the capacity of agencies to care for individual's who are aging in the home and have increasing complex medical needs.
The HCF supports agencies in navigating the medical sector, advocacy, discharge planning, controlled acts training, clinical assessments to evaluate for continuing supports, immunizations, PCR testing, policy advice, and urgent medical referral support.
Yasmina is our Health Care Facilitator and can be reached by email or by phone.
Yasmina is in the office or in the community Monday - Friday, 9am-5pm, except holidays.
